Oftentimes the hospital will give you the necessary paperwork to apply for a social security card for a child when he/she is born. 

However if this wasn't the case, you simply to need to fill out an application, known as Form SS-5.  You can do this online at http://www.ssa.gov/replace_sscard.html, by calling 1-800-772-1213, or by going to your local social security office. 

You'll need one original copy of an identifying document (in this case generally a birth certificate.)  it must be ORIGINAL and not a duplicate.  You need to submit the application at your local social security office and then they will mail you a card, generally in 3-4 weeks.
